PrimeCARE makes managing your health simple, affordable, and proactive. Designed to work alongside a high-deductible health plan, and compatible with HSA or FSA funding, PrimeCARE gives you access to comprehensive preventive and wellness care for one convenient yearly subscription.

• Initial Consultation & Health Risk AssessmentGet started with a full evaluation of your current health and wellness goals.  
• Routine Primary Care VisitsUnlimited access at participating PrimeCARE offices (excluding obstetrics).  
• Preventive Screenings & DiagnosticsIncludes lab work, x-rays (per contract), Pap smears, EKGs, pulmonary function tests, and vision/hearing screenings performed in-office.  
• ImmunizationsRoutine adult flu, tetanus, and pneumonia shots included. Vaccinations for children under age 2 available at cost.  
• Specialized Wellness SupportCounseling on nutrition, diabetes management, and exercise. DEXA scans every 2–3 years when appropriate.  
• Skin Cancer Screenings & Minor ProceduresEarly detection and removal of suspicious lesions (pathology fees not included).  
• Urgent In-Office CareTreatment for minor injuries and lacerations without the ER wait.

PrimeCARE is not an insurance plan.You’ll still need a major medical insurance policy to cover services received outside PrimeCARE offices.
